Telecommunications Work Request


All requests from administrators, faculty, and staff for any moves, adds, and changes to the College's Telephone and Voice Mail System normally requires the completion of an Information Technology Request for Service form. Personnel who are requesting moves, adds, or changes are reminded to plan accordingly to allow ample time to coordinate and schedule the work to be performed. All request will be reviewed for approval by the Director of Telecommunications assigned to the proper staff member for completion. In some cases when a request form is not available, the service request can be directed to the Director of Telecommunications via campus email. The following are some items in which Request for Service form are required:

  • Office Telephone Additions or Moves
  • New Telephone Jacks or Telephone Wiring
  • New Telephone Line Numbers
  • Voice Mail Deletions or Adds (New Employees, etc)
  • New Fax Machines - Require prior approval of Office or Program Directors
  • Cell Phones- Request for Service must contain approval of Division Vice President
